Medication Therapy Management


Medication Pillpacks
Medication pillpacks, or pill containers, are a service we provide for patients that need help in organizing their medications. We fill all medications and sort them appropriately in pillpacks on a weekly basis. As such, we keep track of all medications and refill them when needed. We also include a printout on the pillpack listing the patient's name, address, doctor, medications, strength, and time of day each medication is taken.

Pillpacks are rotated weekly on either Monday or Thursday, and free delivery is available for residents of DeKalb, Sycamore, and Cortland. Initial set up fee includes the purchase of two pillpacks to rotate on a weekly basis, as well as a monthly fee for the service. Have you had your annual medication check up?
A medication check up, also known as medication therapy management or MTM, gives you one-on-one time with a Lehan's pharmacist. After reviewing your medication history, we will sit down and discuss any question or concerns you have about your medication including: prescription drugs, over-the-counter medications, herbal products, and dietary supplements. Working together with your doctor, we will identify any potential problems and ways to keep your medication costs down. In addition to individualized care, you will also receive a personal medication record or PMR to keep for your records and a medication action plan that can empower you to take charge of your health.

You may need a medication check up if you:

  • Use several different medications.
  • Recently lost your prescription insurance.
  • Want to reduce your out-of-pocket medication costs.
  • Have questions or problems with your medications.
  • Have been recently hospitalized.
  • Want to gain control of your health.
